Letter to Year 10 Parents - DTP Vaccinations
It is now time for your child’s Tetanus, Diphtheria, Polio (DTP) and Meningococcal ACWY (Men ACWY) vaccinations!
These vaccines are offfered to all young people in: Year 10 reoffer
Why get your child vaccinated?
• Meningococcal ACWY – protects against four types of meningococcal bacteria (Types A, C, W and Y) that cause meningitis (inflammation of the brain lining) and septicaemia (blood poisoning).
• Tetanus, Diphtheria and Polio (3 in 1 booster) – protects against Tetanus, Diphtheria, and Polio, all of which can be very serious diseases. This is a booster for the vaccinations your son/daughter received as a baby, and then again at approximately 3-4 years (pre-school booster).
If You Need More Information
Please visit http://www.nhs.uk/conditions/vaccinations/pages/vaccination-scheduleage-checklist.aspx for further information about the disease and the vaccine, as well as links to other useful websites.
What do you need to do? It is important every child has a completed form which states if you would like them to receive the vaccinations or not. The online consent form can be found through this link https://www.nottinghamshireimmunisations.co.uk/Forms/DTP
School code: NG135761
Online consent will close at 3.00pm on 7th March 2023.
If you decide you want to withdraw consent you must email sais@nottshc.nhs.uk at least 24 hours before your schools session.
Please Note If we do not receive a completed form: your son / daughter will be given the opportunity to self-consent. The decision to have the vaccination is legally your son / daughter’s as long as they understand the reasons for the vaccination and the issues in giving consent, however, the nurse would much rather have your permission too.
If you have any questions or need support to complete the consent form, please contact our friendly team: sais@nottshc.nhs.uk 07920 182032
